The Primary Care Network brings family doctors, nurse practitioners, emergency department doctors, and community organizations together to coordinate care for everyone in the region.
Our Recent Work
Our Primary Care Network has achieved several milestones in our community, including:
- Launching the CareBridge clinic
- Expanding chronic disease supports, including COPD care
- Using new digital tools to make care faster and better coordinated
- Engaging patients and the community to improve care
These efforts are helping the OHT create a healthier, more connected, and more equitable West Parry Sound.
